Primary responsibility

Engineering and operations leaders decide how much risk the organization will accept, which resources are assigned, whether a change is authorized, and when an incident may move from active response to follow-up work.

Read the evidence state

A leadership readout should answer: what is proven, what remains uncertain, what impact continues, where the candidate boundary sits, who owns the next move, what approval is required, and what happens if the team waits.

Authorize measurable actions

An approval should name scope, intended outcome, rollback, stop conditions, and validation. Avoid approving broad remediation based on urgency alone. When evidence is incomplete, authorize the smallest reversible action that can reduce impact or produce a decisive signal.

Invest after the incident

Use recurring evidence gaps, ownership delays, unsafe manual work, and validation weaknesses to prioritize platform improvements. The goal is not merely fewer incidents; it is faster boundary narrowing and safer recovery when incidents occur.

Expected outputs

Risk posture, authorization decision, resource assignment, operating readout, residual-risk acceptance, and investment follow-up.
